The conversation around immunizations has changed. The science is clear, but a growing wave of misinformation has made it harder for patients and their caregivers to know what to believe. Those questions often follow them into your exam room. And your practice is where vaccine confidence gets built, one conversation at a time.

That’s why we need you.

Program overview:

The AAFP is selecting just 25 practices to join a hands-on initiative designed to help your care team build vaccine confidence in every patient conversation. This practice-level initiative will give your team the tools, training and structure to make every immunization conversation more effective, from point-of-care materials to daily workflow integration.

The goal: Improve immunization rates across all patient populations while giving your team
giving your team the confidence to meet doubt with clarity.

What you'll get.

Evidence-based educational materials for your team and patients to use in the exam room, waiting room and digital channels.
A custom video tailored to your patient population and ready to use across your practice and online platforms. A family physician from your practice will be interviewed at AAFP's Family Medicine Experience (FMX) to bring it to life.
Free FMX 2026 attendance (Oct. 20–24, Nashville), including travel and registration, for you to participate in a prep meeting and be interviewed for your video.

A $2,000 payment to cover administrative costs will be awarded to practices that complete all program milestones.

How it works.

You'll designate a Champion—a physician, resident or team member—to lead the effort. From there, you'll get step-by-step guidance to integrate immunization education into your daily workflow, assess your current immunization processes, and build an action plan for stronger, more confident and consistent conversations at the point of care. Same conversations. Stronger outcomes.

What we'll ask of you.

Designate a practice Champion.
Complete Conflict of Interest and consent forms.
Join a kick-off meeting (August 2026) and regular check-ins via teleconference and email.
Complete a confidence survey on immunization barriers.
Attend a prep meeting and video recording session at FMX 2026.
Have all care team members complete pre- and post-PI surveys.
Integrate educational materials into your daily workflow.
Build and execute a performance improvement action plan.
Complete a post-project survey.
Join a wrap-up meeting (March 2028).

Review and selection:

Applications will be reviewed and participants selected by an internal working team. Selected practices will be notified via email by August 13, 2026.
