Program Overview

Adult immunization rates remain lower than they should be. This initiative gives your practice the tools, framework and expert guidance to change that.

Only 25 practices will be selected. Will yours be one of them?

How it works:

Each participating practice designates a Champion to lead implementation, track progress through chart reviews and drive measurable improvement across the team.

Overall, this program is designed to help practices make adult vaccinations feel less like an extra step and more like second nature.

Review & Selection

Applications will be reviewed, and participants will be selected by an internal working team. The awardees will be notified via e-mail by August 17, 2026.
