Program Overview
Adult immunization rates remain lower than they should be. This initiative gives your practice the tools, framework and expert guidance to change that.
What you’ll gain:
- A structured, evidence-based approach to integrating vaccination into routine care.
- Practical tools to help your care teams assess immunization status and make strong recommendations.
- Streamlined documentation and administration processes to fit within your existing workflow.
Only 25 practices will be selected. Will yours be one of them? The deadline to apply is August 10.
How it works:
Each participating practice designates a Champion to lead implementation, track progress through chart reviews and drive measurable improvement across the team.
Overall, this program is designed to help practices make adult vaccinations feel less like an extra step and more like second nature.
Participating practices will receive $1,000 to cover administrative costs associated with the project, based upon completion of project milestones.

Review & Selection
Applications will be reviewed, and participants will be selected by an internal working team. The awardees will be notified via e-mail by August 13.
