That's why the AAFP has two immunization initiatives designed to meet your practice where it is.

Whether you're focused on strengthening immunization conversations across all patient populations or improving immunization rates for adults 50 and older, there's a program built to help your team make every conversation count.

The New Immunization Conversation

Supported by an unrestricted grant from GlaxoSmithKline.

For practices ready to build vaccine confidence, one patient at a time. This hands-on initiative meets your care team where you work, giving you the tools, training and support to make every immunization conversation land, from point-of-care materials to daily workflow.

How it works:

You'll start by naming a physician, resident or team member, to lead the way. From there, we'll walk you through step-by-step guidance to weave immunization education into your daily workflow, assess where you are today and build an action plan for stronger, more confident conversations at the point of care. Same conversations. Stronger outcomes.

  • Performance improvement program
  • Just 25 practices selected
  • Custom patient education video, filmed at FMX 2026
  • Free FMX 2026 attendance (Oct. 20–24, Nashville), including travel and registration
  • $2,000 administrative payment upon completion
  • 8-month intervention period
  • Focus: all patient populations

What we'll ask of you:

Once your practice is selected, we'll partner with you on the following:

  • Name an Office Champion.
  • Complete Conflict of Interest and consent forms.
  • Join a kickoff meeting (August 2026) and check in with us along the way by teleconference and email.
  • Complete a confidence survey on immunization barriers.
  • Attend a prep meeting and video recording session at FMX 2026.
  • Have your care team complete pre- and post-PI surveys.
  • Weave educational materials into your daily workflow.
  • Build and carry out a performance improvement action plan.
  • Complete a post-project survey.
  • Join a wrap-up meeting (March 2028).

Improving Immunization Culture

Supported by an unrestricted grant from Pfizer.

For practices focused on strengthening immunization care for adults 50 and older. This quality improvement program pairs your care team with an Office Champion, walking you through every step from baseline chart review to implementation to post-measurement, so every patient over 50 gets the care they deserve.

How it works:

You'll start by naming an Office Champion to lead the effort, someone from your care team ready to drive change. From there, we'll walk you through a five-month journey together, from baseline chart review to implementing your action plan to post-measurement, so every step feels doable and every result feels earned. Stronger practices. Better care for every patient over 50.

FAQs

Can my practice apply for both programs?
Yes. Practices are welcome to apply for one or both. If your practice serves a wide range of patients and has a focus on adults 50 and older, both programs can work side by side.

Do I have to be an AAFP member to apply?
Yes, applicants must be active AAFP members. Not a member yet? Join here.

Can I apply on behalf of my whole practice?
Yes, applications are submitted at the practice level, not the individual level. One physician typically leads the application, but the whole care team participates.

What's the difference between the two programs?
The New Immunization Conversation (GSK) focuses on all patient populations and includes a custom patient education video plus FMX 2026 attendance. The Improving Immunization Culture focuses specifically on adults 50 and older and uses an Office Champion model. See the comparison chart for a side-by-side look.

What kind of materials will we receive?
Both programs provide evidence-based educational materials for your care team and patients, tailored to their focus population. GSK includes a custom patient education video filmed at FMX 2026.

Will we have to change our workflow?
Both programs are designed to weave into your daily workflow, not disrupt it. You'll get step-by-step guidance from the AAFP team along the way.

Are there surveys or reporting requirements?
Yes, both programs include pre- and post-project surveys along with regular check-ins. This helps us measure impact and continuously improve the initiatives.

When do the programs start?

  • The New Immunization Conversation: Kicks off August 2026 and wraps in March 2028.
  • Improving Immunization Culture: Baseline measurement goes back to Oct. 1, 2025, with implementation and post-measurement running through 2027.

What's included in the FMX 2026 attendance for The New Immunization Conversation participants?
Selected practices receive free FMX 2026 registration and travel for one designated physician to participate in a prep meeting and video recording session at FMX (Oct. 20–24, 2026, Nashville).

What if I'm already registered for FMX 2026?
No problem. If selected, your registration and travel expenses will be reimbursed. Contact Pam Carter-Smith for details.

Does the Improving Immunization Culture program include FMX attendance?
No, FMX attendance is not included in the Pfizer program.

Do the funders influence program content?
No. Both programs are developed and led by the AAFP with clinical and educational integrity as our top priority. Sponsors provide unrestricted funding.
