Improve Cognitive Care in Family Medicine Settings

Thank you for your interest in applying for the AAFP Cognitive Care in Family Medicine Settings Quality Improvement Project. 

This project aims to give AAFP members and other primary care clinicians access to an important care pathway that can facilitate the early diagnosis of mild cognitive impairment and dementia. In addition, it will provide the AAFP and the University of Washington with the opportunity to build on this project, with the objective of identifying gaps and addressing barriers that may prevent early detection, evaluation and diagnosis of mild cognitive impairment and dementia within family medicine settings.

The goals of the project are to provide education and training for family medicine settings to integrate cognitive evaluation, diagnosis and management in their daily workflows. The project uses an Office Champions method which is designed to ensure a team-led approach is embedded into the practice’s daily workflow to assist practices at the point-of-care by designating a team member to facilitate the implementation of evidence-based strategies and interventions from this quality and performance improvement model. 

Participants will receive useful tools and resources, assistance from subject matter experts, coaching, and educational videos. Findings will be shared on how the practices use the tools and resources, and what is most effective. This project will qualify for American Board of Family Medicine (ABFM) performance improvement credit, which participants will receive upon completion of the project.

The benefits of the Cognitive Care in Family Medicine Settings Quality Improvement project are to increase the number of cognitive evaluations and diagnoses of mild cognitive impairment and dementia that primary care clinicians provide for their patients. With the objective of utilizing effective interventions, strategies and tools that can help put the training into practice. Practices that complete the required milestones will receive $2,000 to cover administrative costs.

Note: Family medicine practices that have implemented a new EHR within the last 6 months or plan to change their EHR systems in the next 18 months are not eligible.

The application deadline is June 15, 2026. Selected practices will be notified by June 30.
